A Community-Based Adult Day Program in Leominster, MA

CAPS Collaborative is starting an exciting new chapter with the launch of Momentum! Momentum is a community-based adult day program for those seeking empowerment, connection, and opportunity.

This program is a natural next step in CAPS’ mission to support individuals through every stage of life. Before Momentum, students had to leave CAPS at age 22 to get services from other agencies. This can be a challenging transition for many individuals and families.

“We’re often asked why we don’t provide services beyond 22, because that can be a challenging time for students to move into that adult world. So we really wanted to explore opportunities to be able to provide that service.”

— Sheri D’Annolfo, Executive Director

This program is recognized as a Community-Based Day Support (CBDS) initiative by the MA Department of Developmental Services (DDS). Here, Momentum adults will explore meaningful paths, build independent living skills, and prepare for job opportunities.

Sneak Peek: Our New Inclusive Space on Mechanic Street

Momentum will operate out of a beautiful new space on Mechanic Street in Leominster. Participants can look forward to a welcoming, inclusive place where they will learn, connect and grow.

Our new 17,000-square-foot space is under construction and customized to meet participant needs. Highlights include:

  • Teaching kitchen with ADA-compliant equipment and cabinets to learn safe meal prep and build culinary skills.
  • Mock apartment to practice independent living skills like making the bed, doing laundry, and completing household tasks.
  • Fitness center fully outfitted with adaptive equipment to build healthy movement habits.
  • Cafeteria and event space to encourage social time and community.
  • Flexible learning spaces to explore hobbies, new skills, and lifelong learning.

“We’re offering supportive services for adults to learn, to explore meaningful life paths, to be able to have enriching opportunities both in the community and in their homes—teaching life skills as well as exploring employment opportunities. That’s where we’re starting, and we hope to grow from there.”

— Mindy LeBlanc, Adult Program Director

Continuing Relationships, Building New Opportunities

For many families, Momentum is more than a new adult day program. It’s a way to foster long-term relationships. In the past, students ages 18–22 focused on preparing to leave CAPS for their next step. That meant saying goodbye to trusted staff who had supported them for years. With Momentum, adults can continue building skills alongside familiar faces they know and trust.

This launch also brings services closer to home. Many adults have to travel long distances to find programs that meet their needs. Opening Momentum in Leominster, where CAPS already has strong community roots, makes those opportunities easier to reach.
